New Telehealth Patients

If you are new to our practice and being seen by Dr. Mary Ella Carter for you initial consultation via telehealth, please read the following instructions.

1. Please download and complete our new patient paperwork. Please send the completed paperwork to our office, along with a photo of a valid government issued photo identification and your health insurance cards, to our email at drmaryellacarter@gmail.com
2. We may ask for photos to be sent to our email. If you are sending photos from your phone, it is best to send them in size “medium”.
3. We will conduct your telehealth appointment via video or telephone call. Please let us know what method is most convenient and reliable for you.
